A patient was recovering from cholangiogenic sepsis with the necessity of mechanical ventilation due to septic shock. She was extubated three days ago. Worsening tachypnea and decreasing level of SpO2, asymmetrical movements of the thorax (minimal excursions on the left side). Breathing sounds could be heard only apically ventrally. Low inflammatory markers, no fever, without antibiotic therapy.
